EEO Self Survey

Use EEO Self Survey to voluntarily provide race/ethnicity and gender information. The U.S. Equal Employment Opportunity Commission (EEOC) requires some employers to report on the composition of their work force in terms of race/ethnicity and gender within job categories.

If you have access to the online survey, you can voluntarily answer questions that self-identify your race/ethnicity and gender according to the classes established and requested by the EEOC for reporting purposes. The Voluntary Self-Identification Survey screen includes further explanation of the importance and use of this information.  

You have the option of answering the survey questions or declining to take the survey. Once you submit your response, you no longer have access to the survey. Contact your administrator to request access.

To take the voluntary EEO Self Survey:

  1. Click employee | Personal Information | EEO Self Survey.

  2. To decline the survey, select "I understand the reason for this request for voluntary self-identification as stated above and choose to decline.", then click Save.

  3. To complete the survey, select "I understand the reason for this request for voluntary self-identification as stated above and have opted to complete this form.". The Gender: and Race/Ethnicity: fields display.

  4. Select your gender in the Gender: field.

  5. Select Yes if you are a person of Cuban, Mexican, Puerto Rican, South or Central American, or other Spanish culture or origin, regardless of race in the Race/Ethnicity: field, then click Save.

  6. Select No if you are not a person of Cuban, Mexican, Puerto Rican, South or Central American, or other Spanish culture or origin, regardless of race in the Race/Ethnicity: field.

  7. Select your applicable race/ethnicity, then click Save.
