OSHA Form 301

Use OSHA Form 301 when reporting a recordable work-related illness or injury. In Paychex HR Online, this is automated through the Incident Report wizard that you complete each time you click Add Accident.

  1. Click Administration | Employee Information | Accidents/Illnesses.

  2. Click Print OSHA Forms.

  3. Click OSHA Form 301.

  4. Select the case from the Select Case: drop-down menu.

  5. Click Print OSHA Form 301. Paychex HR Online generates a PDF document with the criteria you entered. OSHA's Form 301 Injury and Illness Incident Report displays.
