OSHA Form 300A

Use OSHA Form 300A to create a summary of all work-related injuries and illnesses at your company recorded throughout the year.

Note:

Only complete this form once each year.

  1. Click administration | Employee Information | Accidents/Illnesses.

  2. Click Print OSHA Forms.

  3. Click OSHA Form 300A.

  4. Select the company from the Company: drop-down menu.

  5. Select the location from the Location drop-down menu. This is a required field.

Note:

The Location: field defaults to the first available location set up in Company Setup | List Setup | Location that OSHA checked. If there are no locations available, the field is blank.

  1. Type the date range in the Date Range: fields or click the calendar icon next to the Date Range: fields to select the date range.

  2. Select the branch from the Branch: drop-down menu.

  3. Select the department from the Department: drop-down menu.

Note:

The following information is on OSHA Form 300A, but is not stored by Paychex HR Online. Either populate the information here before printing, or fill out the information manually afterward.

  1. Type the industry description in the Industry Description: field.

  2. Type the Standard Industrial Classification (SIC) in the Standard Industrial Classification (SIC): field or type the North American Industrial Classification (NAICS) in the North American Industrial Classification (NAICS): field, type the annual average number of employees in the Annual average number of employees: field, and type the total hours worked by all employees last year in the Total hours worked by all employees last year: field.

Note:

Use the OSHA Worksheets to calculate the total hours worked by all employees.

  1. Click Print OSHA Form 300A. Paychex HR Online generates a PDF document with the criteria you entered. OSHA's Form 300A Log of Work-Related Injuries and Illnesses form displays.

  2. Write in the data from your OSHA Form 300 onto the OSHA Form 300A.

  3. Post the OSHA Form 300A in a visible location in your workplace. If you have more than one location, you must post a copy of OSHA Form 300A at each facility.
