Print OSHA Forms

Use Print OSHA Forms to enter OSHA Form 300 criteria and print OSHA Form 300. You can also access OSHA Form 300A, OSHA Form 301, and Worksheets & Calculators from this screen.

To view Print OSHA Forms:

1. Click administration | Employee Information | Accidents/Illnesses.

2. Click Print OSHA Forms. The Print OSHA Forms screen displays.

To view OSHA Form 300A, refer to OSHA Form 300A.

To view OSHA Form 301, refer to OSHA Form 301.
